In case of emergency and we are unable to get a hold of parent or guardian, please provide the name of nearest relative/neighbor to be contacted.

I fully understand that by signing this form I am granting my permission for my child to fully participate in any activities sponsored by Peculiar United Methodist Church and carries with it the following:

*My child will observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Children/Youth Directors and/or adult sponsors acting on behalf of Peculiar United Methodist Church.

*I understand that there are certain inherent risks associated with participation in youth activities and accept full responsibility for personal injury to my child and further release or discharge Peculiar United Methodist Church for injury, loss or damage arising out of said activities.

*I agree to pay for all damages to Peculiar United Methodist Church or its property caused by my child’s negligent, reckless, or willful actions

*I agree that I am under no pressure or duress to sign this agreement and that I have been given a reasonable opportunity to review it before signing. I further agree and acknowledge that I am free to have my own legal counsel review this Agreement if I so desire.  I further agree and acknowledge that Peculiar United Methodist Church has agreed to refund any fees I have paid for youth activities if I refuse to sign this Agreement.

*I knowingly release, absolve, indemnify, and hold harmless Children/Youth Directors and/or adult sponsor acting on behalf of Peculiar United Methodist Church and the Peculiar United Methodist Church from all claims that might result from any injury or death of any minor or in the administering of or lack of medical treatment. 

*I hereby authorize any Children/Youth Leader and/or adult sponsor(s) from Peculiar United Methodist Church to obtain any medical attention and/or treatment, which in their opinion is necessary for my child.

*Should medical help be needed, I agree to pay either directly or through my own health and accident Insurance policy all medical or hospital costs and to be solely responsible for said treatment and the cost thereof.

*Should my child need to leave the activity or trip early due to personal reasons or medical attention, I agree to pay all transportation costs associated with said event for the child to return home.

*I hereby verify that my child is in good physical condition and is able to participate in moderate to intense physical and/or fitness activity. 

We are SO excited that you are registering your child for VBS! Dinner will be served at 5:30pm (easy, kid-friendly meals) and the VBS program begins at 6:00pm. This is OUR way of serving our community.  There is absolutely no cost required for participation, we are just excited to serve you and your family!  We have had folks ask about helping; If you would like to volunteer your time, just let us know below and Justine Baron will be in contact with you.  If you would like to help support VBS with a financial contribution, know that it is appreciated but absolutely not required.
